Who Qualifies?

You qualify if you are…

  • Diagnosed with life-limiting illness by physicians
  • Meet the eligibility for end-of-life care under Medicare guidelines
  • Life expectancy is six months or less as confirmed by a physician
    • Not limited to above timeframe as long as the individual meets hospice eligibility

  • Medicare
  • AHCCCS
  • Most Insurance

Care is not denied as long as individual meets Medicare guidelines for hospice eligibility. All services are covered under Medicare as long as the need for the services is related to the patient’s hospice diagnosis.

WHAT IS HOSPICE?

Hospice care is the collaboration of a dedicated team of healthcare professionals who provide comprehensive end-of-life care to those with life-limiting illnesses at their place of residence, whether that may be a private home, a skilled nursing facility, assisted living community or assisted living care home. Tranquility Hospice offers short-term inpatient care for patients who require around-the-clock nursing care for pain and symptom management. Hospice also provides respite stays for patients who need a place to go to so that their family members may have a short break from care giving. Hospice care is available 24 hours a day 7 days a week. The hospice team is available for care and support after hours and on the weekends.
